Semi-Independent Homes

We Raise Champions!

Here are the advantages that stand us out

RISK MATCH / REFERRALS Advantage of risk matching to Hope Quality Care home is that it helps to ensure that the level of care provided is appropriate for each resident's individual needs. By matching residents with support givers who have experience and training in the specific areas required. Hope Quality Care Home can provide a higher standard of care and support. This personalised approach can lead to improved health outcomes, increased satisfaction for residents and their families, and, ultimately, a stronger reputation for the facility. We also ensure that our referrals are carefully reviewed before making a decision about possible admissions, and the input of other existing residents is not disregarded. The referral was originally addressed via telephone; it is imperative to dispatch a follow-up email that includes the referral particulars and, if pertinent, the risk assessment. In the referral, we will incorporate the youth's age, gender, and any safety concerns, along with comprehensive details about their background and life experiences. This information will help our team assess the youth's needs and determine the appropriate placement for them. Additionally, please provide any relevant contact information for the youth or their guardian so that we can schedule an initial meeting to further discuss their situation. The subsequent stage involves Hope Quality Care Limited performing a risk assessment and connecting the referred young adults with a placement agency. We also encourage placements, unit visits, or tours if needed. The aim is to ascertain any potential sources of concern based on the facts presented in the referral. We will reply to you promptly, within a few hours at the latest, to confirm receipt of the recommendation and assess our capacity to meet the young adult’s requirements. Should the requirements remain unfulfilled, Hope Quality Care Limited will terminate the referral and furnish a written explanation. We will provide a reason for rejection if we are unable to offer a placement. Care Place, a pioneering procurement platform, submits referrals. Our Placements Manager of Hope Quality Care Limited will display the specifics of the selected unit and respond accordingly, and our placement manager will subsequently access the online risk assessment and any accompanying recommendation tools. This process ensures that each young adult is matched with a placement that meets their individual needs and goals. Our team is dedicated to providing the highest quality of care and support, and we strive to create a safe and nurturing environment for all individuals in our care. By carefully assessing each referral and placement, we can ensure that each young adult receives the personalised care, support, and attention they deserve. Our goal is to empower and uplift the young adults in our care, helping them to reach their full potential and lead fulfilling lives. A placement risk assessment and risk matching procedure will be conducted, and an offer will be made if considered for placement. The young adult's previous placement details will be requested in completing our admission logs, and our placement induction, etc., will not be disregarded. Additionally, our team will work closely with the young adult's support network to ensure a smooth transition and provide ongoing support throughout their stay. Any special requirements or accommodations needed will be taken into consideration to ensure the individual's needs are met. Our goal is to create a safe and supportive environment where the young adult can thrive and reach their full potential. Our dedicated staff will be there every step of the way to offer guidance, support, and encouragement. On this note, we will use this opportunity to say thank you for entrusting us with this referral; we look forward to working with you to provide the best possible care and support for the youths in need. Finally, we understand the importance of collaboration and communication with all parties involved in the individual's care. We will regularly update you on their progress and address any concerns or changes in their support plan. Our team is committed to providing comprehensive and personalised support to ensure the young adult's well-being and success. Together, we can make a positive impact on their journey towards independence and a brighter future.

SUPERVISION, TRAINING & DEVELOPMENT

We select personnel who possess the essential skills and training via our safer recruitment process and offer them training that aligns with the values of Battersby House and the requirements of our service users. A formal programme of staff supervision underpins the Staff Performance Appraisal process. Staff training programs are categorised into three distinct groups: 1. Induction Training - for all staff members, adhering to relevant regulatory requirements, including safeguarding and child protection procedures. 2. Job-specific Training - specific training required for each job position, which may encompass specialised components such as C.O.S.H.H. awareness, restorative justice, safe interventions, and other relevant training as necessary. 3. Refresher Training - for all staff members. The system is organised to ensure that a documented Training Plan exists for every Job Position outlined in the Home’s Organisation Chart. The Training Plan will be tailored to the needs of the Job Position and focused on enabling the job holder to effectively perform the responsibilities outlined in the relevant Job Description. Every Training Plan is crafted to emphasise two essential components of job training as outlined below: ● A compilation of the necessary training components, presented as distinct Training Modules (e.g. a course on Behavioural Management or Health & Safety). ● A compilation of the Policies that are directly relevant to the Job Position. The Training Plan includes a section to outline the relevant policies (Staff Matrix) and requires the job holder to confirm that they have read and understood the documents. Upon recruitment, the new employee will be provided with a copy of the relevant job description along with a copy of the corresponding training plan that outlines the overall training requirements. The job holder, along with the appropriate Supervisor, will complete the Training Plans, detailing any relevant training the employee has received in previous employment. The unfilled elements indicate the Training Modules that need to be completed for the individual to create a personal Training Plan. Training Plans are organised in a way that they also serve as a comprehensive Training Record for the employee, as the Forms include sections to document training details. Whenever training is conducted, the relevant details will be recorded, and the employee will sign, accompanied by a countersignature from the Supervisor in the designated spaces. Subsequently, personalised Refresher Training is provided based on specific requirements: 1. When performance appraisal has highlighted a requirement. 2. Considering new legislation, regulations, standards, or marketing initiatives. • Every employee will have their training records, and it is the responsibility of the employee's immediate supervisor to maintain these records. The management of the home is dedicated to a continuous program of staff training and development. The Home Manager will oversee the management of the home, supervise the senior key workers, and spearhead the workforce training strategy in collaboration with the stakeholders. Training for staff at all levels is provided both internally and through external resources. Staff have access to the online training portal “Training Hub, Social Care TV,” which offers a program for training, reflective practice, and management of supervision. Staff development is a continuous journey, and we have established various methods to guarantee that we function as a learning organisation, incorporating insights from essential professionals. • Training and supervision • Team meetings • Consultation for group and team embedding from therapists • Collaborating as part of the team • Key working and joint working with colleagues - mentoring and modelling good practice • Collaboration with other professionals and agencies • Additional duties and responsibilities assigned to staff as they gain skills and confidence within the team • Team-building days • Insights gained from feedback, both positive and negative • Research findings communicated effectively
